Therapy services are offered to clients who are physically located in the state of Texas at the time of service.
Notice of Privacy Practices (HIPAA)
Mind on Mend, PLLC is committed to protecting the privacy of your protected health information (PHI) in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA), applicable federal regulations, and Texas state law.
PHI includes information that identifies you and relates to your mental health care, treatment, or payment for services. I may use or disclose your PHI for purposes of treatment, payment, healthcare operations, and as otherwise permitted or required by law.

A full Notice of Privacy Practices is available upon request and is provided during the intake process. You may also request a copy at any time.
If you believe your privacy rights have been violated, you may file a complaint with Mind on Mend, PLLC or with the U.S. Department of Health and Human Services Office for Civil Rights. Filing a complaint will not affect your care in any way.
Notice to Consumers – Texas Mental Health Services
In accordance with Texas law (House Bill 4224 and Section 181.105 of the Texas Health and Safety Code), the following information is provided to help consumers understand their rights and available resources.
How to Request Your Health Care Records:
You have the right to access your health care records. To request a copy of your records, submit a written request to the practice or your treating clinician. Requests may be made by email or through the secure client portal.

Records are provided in accordance with Texas law and applicable privacy regulations. Records from services provided through a prior employer or third-party platforms must be requested directly from those entities.

How to File a Complaint:
The Texas Behavioral Health Executive Council (BHEC) regulates licensed mental health professionals in Texas. Consumers may contact BHEC with questions about licensure or professional standards, or to file a complaint.

Good Faith Estimate Notice
You have the right to receive a Good Faith Estimate explaining how much your mental health care services will cost.
Under federal law, health care providers must provide an estimate of expected charges for services to clients who are uninsured or who choose not to use insurance.

You have the right to receive a Good Faith Estimate for the total expected cost of services before scheduling or upon request. If you receive a bill that is at least $400 more than your Good Faith Estimate, you may dispute the bill.

Fees and Financial Transparency
Mind on Mend, PLLC provides private-pay mental health services. Insurance is not accepted at this time.
Current fees are as follows:
Individual therapy sessions: $150 per session
Additional fees may apply for:
Late cancellations or missed appointments
Extended sessions or additional services requested outside of standard therapy sessions
Fee information, cancellation policies, and payment expectations are discussed prior to the start of services and are outlined in the informed consent documentation. Clients are encouraged to ask questions about fees at any time.
